T3 Online Hearing Screener Course

NOTICE:  The T3 Online Course is being phased out.  If you wish to complete the course, you must send in your application and payment no later than December 1, 2012.

Those currently in process must complete the course including the practical exam NO LATER THAN December 20, 2012.  If you have any questions, please contact Celeste Burrell at burrellc@cals.arizona.edu.

This course is for individuals who need to renew their approval by the Arizona Department of Health Services (ADHS) as a T3 Hearing Screener. Successful completion of this course meets the requirements for renewal of training as set forth in the ADHS Rules and Regulations for Hearing Screening. The course takes 4 hours to complete. Cost: $50 payable by check or money order.

To successfully complete the course, you must:

1. Read the slide presentation
   
2. Complete the course-required assignments
   
3. Pass a multiple choice and true/false test administered via the internet
   
4. Pass a practical exam with a designated T3 trainer that demonstrates your screening competency
